Terms of membership
What you get, what you pay, how to leave. Written to be read, not to be survived.
1. Who you are dealing with
Tab is operated by HeydayMD LLC, a New Mexico limited liability company ("Tab", "we"). Tab is a technology and membership company. We do not practice medicine. Medical care is provided by independent, licensed clinicians through Tab's clinical partner, and those clinicians alone make clinical decisions. Prescriptions are dispensed by licensed pharmacies. Laboratory testing, where offered, is performed by certified laboratories.
2. What membership includes
- Access to primary-care visits with licensed clinicians in your state, as described on the plan you purchase.
- $0 pricing on the medications listed on the covered generic list, at participating pharmacies, while your membership is active.
- Reduced pricing on a wider set of medications that are not on the $0 list.
Membership does not include emergency care, hospital care, imaging, surgery, controlled substances, brand-name or specialty medications, or any service a clinician determines is not clinically appropriate.
3. The covered list can change
The $0 medication list depends on our pharmacy partners' supply and pricing. Medications may be added or removed and pricing may change. We will not remove a medication you are actively filling without notifying you first. A specific medication is never guaranteed: only a licensed clinician can decide what to prescribe, and no membership entitles you to a particular drug.
4. Billing, renewal and cancellation
- Membership is billed in advance, monthly or annually depending on the plan you choose, to the payment method you provide.
- Membership renews automatically at the then-current price until you cancel.
- You can cancel at any time by emailing hello@tabprimary.com or from your account. Cancellation stops the next renewal.
- Cancelling ends access and $0 pricing at the end of the period you have already paid for. We do not pro-rate a partial month or year.
- If a payment fails we may suspend membership pricing until it is resolved.
- Promotional codes apply as stated at the time of purchase and are not retroactive.
5. Prepaid add-on programs
Some clinical programs, including hormone care for women, are sold on a prepaid 3-month, 6-month or 12-month term. They are not available monthly. You may buy such a program on its own without a Tab membership, or as a discounted add-on if you already have, or sign up for, a Tab membership.
Standalone pricing does not include a Tab membership and does not include the $0 or discounted medication pricing described in section 3. You pay your pharmacy’s cash or insurance price for any medication your clinician prescribes. The discounted add-on rate is only available together with an active Tab membership, and is charged in addition to your membership fee.
- The full term is charged at purchase and the term you have paid for runs to its end.
- Cancelling stops the next renewal. It does not refund the remainder of a term already paid, and we do not pro-rate an unused part of a term.
- Program terms renew automatically at the then-current price unless you cancel before the renewal date. Where the law requires it, we will send you a renewal reminder before we charge you.
- An add-on and your Tab membership are billed and cancelled separately. Cancelling your membership ends your eligibility for the discounted add-on rate and the $0 medication pricing going forward, but does not refund a prepaid add-on term already paid. Cancelling an add-on does not cancel your membership.
- The add-on pays for clinical care — the visits, the prescriber relationship and ongoing management. It does not pay for medication, and laboratory testing is not included and is billed separately.
- An add-on does not entitle you to any particular prescription. Your clinician decides what, if anything, is appropriate for you, and may decline to prescribe.
6. Eligibility
You must be 18 or older and physically located in a U.S. state or territory where our clinical partner is licensed. Adults may add household members to a family plan; you may only add a minor if you are that minor's parent or legal guardian. Availability of specific services varies by state, and some states regulate discount plans in ways that limit or change what we can offer there.
7. Your responsibilities
- Give accurate and complete health and identity information to your clinician.
- Do not share your account or use another person's membership.
- Do not use Tab for an emergency. If you have a medical emergency, call 911.
8. No guarantee of outcome
Nothing on this site or in any communication from us is a promise of a diagnosis, a prescription, a result, or a specific saving. Statements about savings are estimates based on typical retail pricing and will differ for you.
9. Suspension
We may suspend or end a membership for non-payment, fraud, abuse of staff or clinicians, attempts to obtain medication improperly, or violation of these terms. Where we end a membership without cause, we refund the unused portion of a prepaid term.
10. Limits of liability
Tab provides the membership platform "as is". To the maximum extent permitted by law, Tab's total liability arising out of the membership is limited to the fees you paid in the twelve months before the claim. This limit does not apply to the professional liability of the clinicians or pharmacies, who are responsible for their own care and dispensing, and nothing here limits any right you cannot waive under law.
11. Disputes and governing law
These terms are governed by the laws of the State of New Mexico, without regard to conflict of law rules. We would much rather fix a problem than argue about it — write to hello@tabprimary.com first.
12. Changes
We may update these terms. Material changes affecting price or cancellation will be sent to your email address before they take effect, and continuing your membership after that date means you accept them.
